Anovulation Explained Without the Jargon

You track your cycle. You get a period every month. You assume everything is working. But if you are trying to conceive and nothing is happening, you might be dealing with the silent fertility thief: Anovulation.

What is Anovulation?

Simply put: Your ovaries did not release an egg.

  • But I got a period?
  • Yes. You likely had “Withdrawal Bleeding.” This happens when estrogen builds up the lining, drops slightly, and the unstable lining sheds. It looks like a period, but biologically, it isn’t a true menstrual cycle because no progesterone was produced.

Why Does It Happen?

The brain and ovaries are constantly talking. Anovulation means the phone line was cut.

  1. PCOS (Use PCOS): The ovaries are “loud.” Too many follicles start growing, testosterone rises, and the dominant egg gets stuck.
  2. HA (Hypothalamic Amenorrhea): The brain is “scared.” Stress, undereating, or over-exercising tells the brain it is not safe to reproduce. It shuts down the signal.
  3. Thyroid Issues: A sluggish thyroid slows down the recruitment of the egg.
  4. High Prolactin: Elevated prolactin (milk hormone) directly blocks the ovulation signal.

How to Detect It

You cannot tell just by bleeding. You need data.

  1. No Temperature Shift: If you track Basal Body Temperature (BBT), an ovulatory cycle has a distinct temp rise in the second half. An anovulatory cycle is flat.
  2. Low Progesterone: A blood test on Day 21 (or 7 days before your period) will show very low progesterone (< 3 ng/mL).
  3. OPKs are Confusing: You might get “almost positive” lines all month (common in PCOS) or no surge at all.

The Fix

The good news? Anovulation is one of the easiest fertility problems to treat.

  • The Lifestyle Fix: For HA, eat more. For PCOS, manage insulin.
  • The Meds: Letrozole or Clomid are pills that trick the brain into shouting louder at the ovaries. They have a very high success rate for inducing ovulation.

Don’t guess. If you suspect your cycles are “fake,” get the Day 21 Progesterone test to know for sure.
