Postpartum Lab Checks You Should Discuss at 6-12 Weeks

The “6-week checkup” is often a disappointment. The doctor checks your stitches, asks if you are depressed, discusses birth control, and sends you on your way.

But internally, your body is still in chaos. To truly recover, you need data. Here are the blood tests you should advocate for at this appointment.

1. Complete Blood Count (CBC) + Ferritin

Why: You lost blood during delivery.

  • The Risk: Iron Deficiency Anemia. It causes extreme fatigue, dizziness, hair loss, and increases the risk of PPD.
  • Target: Hemoglobin > 12 g/dL. Ferritin > 50 ng/mL.

2. Thyroid Panel (TSH, Free T4)

Why: 5-10% of women develop Postpartum Thyroiditis.

  • The symptoms: Rapid weight loss/anxiety (Hyper) or weight gain/depression (Hypo).
  • The Confusion: These symptoms are often dismissed as “new mom stress.” A simple TSH test clarifies it.

3. Fasting Glucose / HbA1c (If you had GDM)

Why: If you had Gestational Diabetes, you are at high risk of it persisting as Type 2 Diabetes.

  • The Rule: You must be re-tested at 6-12 weeks. Do not skip this.

4. Vitamin D and B12

Why: Pregnancy depletes these stores to build the baby’s bones and nervous system. Breastfeeding continues to drain them.

  • The Risk: Low D leads to bone pain and low immunity. Low B12 leads to brain fog and nerve issues.
